Form W-9 is how a U.S. person gives their correct taxpayer identification number (TIN) — a Social Security Number or an Employer Identification Number — to someone who has to report payments made to them to the IRS. Businesses usually ask for it before paying a freelancer, contractor or vendor.
You fill it in when a payer asks for it, and you give it back to that payer. It is not sent to the IRS. People who are not U.S. persons generally use a Form W-8 (such as W-8BEN) instead.
Click each box and type: your name (Line 1), business name if different (Line 2), federal tax classification (Line 3), address (Lines 5–6) and your TIN in Part I. Then download the filled PDF and sign Part II.
